Steps To Apply:
- You must be admitted in a SNF or Hospital for 60 consecutive days to qualify.
- Return Referral Packet. (Current wait time is 12 months)
- Return LIC602A from your primary care physician.
- After 60 days you may leave the SNF or Hospital but must notify our office.
