2. Return LIC602A from your primary care physician.
3. If currently residing in an assisted living facility, return the LIC603 document.
4. Submit if Applicable:
   Designated Power of Attorney (DPOA)
   Advanced Healthcare Directive (AHCD)
   Proof of State Conservatorship
