Adult Protective Services & Ombudsman

Steps To Apply:

1. Submit an active APS or Ombudsman case referral on county letterhead
2. Return Referral Packet. (Current wait time is 1-2 months.)
3. Return LIC602A from your primary care physician.
4. If currently residing in an assisted living facility, return the LIC603 document.
5. Submit if Applicable:
   Designated Power of Attorney (DPOA)
   Advanced Healthcare Directive (AHCD)
   Proof of State Conservatorship
